IT Services Online Forms

IT Services provides a number of accounts and resources for faculty and staff use. For new employees, supervisors should complete and submit an account request form at least 5 days prior to employment start date.

PDF forms should be completed on your computer, printed and (if necessary) signed. See complete instructions below.
Delays in account creation can be expected just before fall semester due to the high number of requests.

 


Service Requests

IT Services routinely provides network software installation services for departmental applications in the student-access labs, on the faculty/staff network, or on the Banner server -- complete the appropriate form and submit with the software. For other new systems, labs, or changes to existing systems you may request services by completing the IT Services' Service Request. IT Services' Service Request Form Includes:

  • Requests for New IT Systems
  • Requests to Enhance IT Systems
  • Requests Requiring New Hardware or Software
  • Design and/or Construction of Computer Lab
  • Add New Network Area

Banner/IU Admin/PU Admin

  • Once your Banner account has been created the Registrar's office will be notified. You will then be contacted about training dates and times.
  • Small group hands-on training sessions are held as needed for new Banner users. All sessions are held in KT G13 and last approximately 2 hours.
  • IU Admin. Systems User Agreement form required (Return form to Help Desk with new account form or modify account form)
  • Purdue Admin. Systems User Agreement forms 182 & 183 required (Return form to Help Desk with new of modify form)

PDF Form Instructions

  • At your computer, complete the PDF form.
  • Please verify the information for accuracy (especially external e-mail addresses/phones).
  • Print the page and obtain a signature
  • If PDF forms do not print properly, try the following: File > Print > Advanced > "Print as image" > OK > OK.
  • Account requests may take up to 5 days.
